There is a brand called 'Buffalo' that make children's road gear. The only place I have found in New Zealand that stocks them is in Foxton, but I can't remember what they are called. They sometimes sell the gear through TradeMe. I have also found the Buffalo stuff online from the UK, but haven't ordered from there myself. Looks like pretty good gear in a wide range of sizes.

motomail and cycletreads often clearing out stuff- frustratingly tho its often at at the extremes of the sizing scale.
ie - XXS helmets.
i bought some el cheap XXS stuffs for the kids when they first started on the back, aged around 5 or 6. Replaced it after about 3 years with fairly cheap XS and S sized stuff, which again lasted 3 or 4 years.
Didn't lay out heaps of money - not a reflection on the value i place on my kids, just a lack of funds to Invest in gear.
Take it easy and slow with kids on the back ...and the chances of being in an accident are reduced.
