    No mechanical failure, just braking too hard. Go out in a wet car park and brake harder until it locks and skids, it's easy to catch at low speed. Happens all the time, watch motogp in the rain, always at least a couple people losing front on the brake.

    Or just ride a bicycle on wet grass...
    ...Full throttle till you see god, then brake.
