Yes, clearly the issue with his modem connecting slowly and his line having high attenuation is all down to the choice of OS on the computer attempting to use the connection.

Get yourself a decent modem to start. Even if it doesn't help things initially you will benefit down the line.

If it's consistently connecting at 64kbps every time you reconnect, it seems fairly likely it's due to the throttling system. If you haven't exceeded your cap, it's possible your ISPs automated system is malfunctioning, so if you call them again, you might want to mention it to them and get them to make sure that isn't the case.

As far as the new modem goes, the version of ADSL is unlikely to make any difference to this specific problem unless there's an issue with the DSLAM in your cabinet, but it is possible your old modem is just giving up the ghost, and either way, an ADSL2+ compatible modem should allow better speeds if you're not too far from the cabinet.

Even if your computer is not at fault, I'd be doing a full malware check on all computers, to see what kinds of nasties have invaded your firewall. Some of the pesky blighters use your connection to download more of themselves, so it pays to get rid of them while you can.

Just curious, what's the average dl/ul speed? I'm getting below:

DownStream Connection Speed: 2190 kbps
UpStream Connection Speed: 65 kbps

I could flick something to make upstream faster by sacrificing some download speed, but that's all the speed I get. I used to get over 3000kbps but don't know where that's gone.

Do equipment swaps along with p.dath's stuff, sometimes it's a tired modem, other times it can be an unfiltered device or even the filters are crap. New filters, no problem, seen a lot of random things before.

What brand of ADSL modem? One of our customers has switched from D-Links as they don't seem to play nicely with ADSL2. They were fine prior to the netork upgrades.
