Your S/N margin on your line is shite. Ignore waffle about ISP throttling...even if they throttle your download your modem will still connect at the full rate; the "throttling" is done further up the food chain. It's likely your modem's minimum handshake* speed is 64kbps. P.Darth is pretty much on the money.
a) Unplug all additional phone equipment to help isolate the issue (do you have monitored alarm etc?....also check if your SkyTV box is plugged into the phone line).
b) Plug in your ADSL modem WITHOUT a line filter (RJ11 to BT jack...."borrow" a line from one of your handsets)
c) Check the speed, if crap, try another jackpoint.
d) Check the speed, if crap, replace line from jackpoint to modem.
e) Check the speed, if crap, ask ISP to do a line test and "port reset" (may take an hour)
f) If crap, repeat above steps with a different modem (very unlikely)
g) If still crap, risk the 80 bucks or whatever and get your ISP to send out a Chorus tech. While you're at it, spend the extra peso's and get them to put in a proper filter (a.k.a. "splitter").
Upstream rate being way better than your downstream rate normally indicates an actual wiring fault. Might have been caused by dodgy sparky, a power spike or more often than not some dodgy home-made phone extensions purchased from Bunnings or wherever. If you don't pay the "wiring and maintenance charge" then my best advice is to ring up Telco, request it and then wait a couple of weeks and report the fault
*Modems start by trying to "handshake" or establish a reliable connection at full speed. If a line is noisy, they lower the speed until they can establish a reliable connection or "handshake".
