From the research I've done it would appear that a vast number died from a combination of malnutrition and disease.

However, if you have a chamber with say 30-40 people desperate to get out (cause they are being murdered) which way do you want the door to open? Opening outwards where the combined weight of the people could potentially bust the door open? Or do you want it opening inwards so that the harder they try to get out, the more difficult it is.
