I don't know any more than what is reported in the newspaper, but I would hazard to guess this incident involved red (or amber) light running by one of the two parties involved.

My thoughts exactly and an intersection well known for it. I’m through there lots and usually have one hand steering and one hand hovering over the horn if I’m at the front of the queue.

I was through there on my push bike only 30 min earlier.

Even here in Dunedin ... I've lost count of the number of times MY light goes to green ... and there's still traffic going through the intersection.

And when the shit hits the fan ... somebody dies ... everybody's like ... it was an accident ... it wasn't their fault .. !!!

It was the sucker ... that waited until the green to go before they moved fault.

More red light cameras ... soon ... or sooner ...

There's an intersection near my place it's normal to have to sit on green waiting for people who ignored the red. They are not the main worry though, it's the odd one who comes through on the main road at plus or minus 100kph. Red light? What red light?

Yes, those are the ones that normally do the greatest damage.

I can only improve upon my own errors, but on a 90 minute ride yesterday I counted 6 cars that just flew through a stop sign as it didn't exist. I was on the main road, they were each at an intersection in front of me (approaching the intersection) on my left or right.

There is no way they didn't see me. But chose to do as they wished.

Interesting that one of the fatal accidents mentioned ... the rider was found minus his helmet. That was found at the neighboring property. How does a properly fitted helmet "Fall Off" ... ??

Rusted strap mounts that fail with an impact?

Hidden behind the foam they never get checked.

Could be a host of other reasons, though.

Straps not done up (just put the helmet on but not secure it)? Humans are very imaginative as far as doing dumb stuff is concerned.

Another reason why you should replace your helmet on a regular basis ...

People should treat their helmets ... like their life depended on them ...

And 10/15 year old tyres even if they have plenty of tread. Life depends on them too.
